Facility Contact Form: This document is filled out by the facility and keeps a copy for their records, and Rocky Mountain R.E.A.D. keeps a copy. This document describes some of the following:

R.E.A.D. School Program Agreement : A clear understanding of between our organization and R.E.A.D. teams, and the facility and staff helps everything run smoothly. With this in mind, this document states what is expected from each and is read and signed by all parties involved.
